Medical paternalism has long been a common medical practice. However, patient autonomy in healthcare has been recently adopted by doctors and patients alike. This study explored whether doctors and patients in a tertiary care hospital in Saudi Arabia preferred autonomy or paternalism in shared decision‐making. A total of 118 participants (51 patients requiring total knee replacement, owing to stages 3–4 of osteoarthritis, and 67 doctors) from the Eastern province, Saudi Arabia. responded to a 17‐question category‐based questionnaire involving four scales of (...) autonomy. Descriptive statistics and chi‐square test results revealed that in this hospital, patients preferred a paternalistic approach toward their medical care along with a full disclosure of the risks related to surgical procedures. We recommend health education regarding the specific autonomy subscales (doctor knows best, patient should decide, right to non‐participation, and obligatory risk information), and the implementation of protocols that protect patients' rights and enhance personal autonomy. W.L. Craig has argued that the universe has a beginning because (1) the infinitude of the past entails the existence of actual infinite multitudes of past intervals of time, and (2) the existence of actual infinite multitudes is impossible. Puryear has rejected (1) and argued that what the infinitude of the past entails is only the existence of an actual infinite magnitude of past time. But this does not preclude the infinitude of the past, Puryear claims, because there can be (...) no justification for the claim that actual infinite magnitudes are impossible. I argue, against Puryear, that there can be such a justification. I claim, nevertheless, that, for reasons entirely different from Puryear’s, the finitude of the past cannot be established based either on the impossibility of actual infinite multitudes or on the impossibility of actual infinite magnitudes. Breaking bad news (BBN) to parents whose newborn has a major disease is an ethical dilemma. In Saudi Arabia, BBN about newborns is performed according to the parental preferences that have been reported from non-Arabic/non-Islamic countries. Saudi mothers' preferences about BBN have not yet been studied. Therefore, we aimed to elicit the preferences of Saudi mothers about BBN concerning newborns. We selected a convenience sample of 402 Saudi mothers, aged 18-50 years, who had no previous experience with BBN. We selected (...) them via a simple number-randomization scheme from the premises of a level III Saudi hospital between October of 2009 and January of 2011. We used a hypothetical situation (BBN about trisomy 21) to elicit their preferences about BBN concerning newborns via a structured verbal questionnaire composed of 12 multiple-choice questions. We expressed their preferences as percentages (95% confidence interval), and we used the Kendall's W test (W) to assess the degree of agreement in preferences. The Saudi mothers preferred that BBN be conducted with both parents together (64% [60-69]), albeit with weak levels of agreement (W = 0.29). They showed moderate agreement in their preferences that BBN should be conducted early (79% [75-83], W = 0.48), in detail (81% [77-85], W = 0.52), in person (88% [85-91], W = 0.58), and in a quiet setting (86% [83-90], W = 0.53). With extremely weak agreement, they preferred to have a known person present for support during BBN (56% [51-61], W = 0.01), to have close bodily contact with their babies (66% [61-70], W = 0.10), and to have no another patients present (64% [59-68], W = 0.08). They showed moderate levels of agreement in their desires to detail, in advance, their preferences about process of BBN by giving a reversible, written informed consent that could be utilized for guidance, if needed (80% [76-84], W = 0.36). In our experience, Saudi mothers' preferences about BBN concerning newborns are varied, suggesting that a "one-size-fits-all" approach is inappropriate. A reversible, written informed consent detailing their preferences about BBN that would be kept in their medical records and utilized for guidance, if needed, may be the best solution, given this level of diversity. The pressures that led to the evolution of episodic memory have recently seen much discussion, but a fully satisfactory account of them is still lacking. We seek to make progress in this debate by taking a step backward, identifying four possible ways that episodic memory could evolve in relation to simulationist future planning—a similar and seemingly related ability. After distinguishing each of these possibilities, the paper critically discusses existing accounts of the evolution of episodic memory. It then presents a novel (...) argument in favor of the view that episodic memory is a by-product of the evolution of simulationist future planning. The paper ends by showing that this position allows for the maintenance of the traditional view that episodic memory operates on stored memory traces, as well as explaining a number of key features of episodic memory: its being subject to frequent and systematic errors, its neural co-location with the capacity for simulationist future planning, and the potential existence of non-human episodic memory. We investigate large cardinal axioms beyond the level of ω-huge in context of the universality of the suitable extender models of [Suitable Extender Models I, J. Math. Log.10 101–339]. We show that there is an analog of ADℝ at the level of ω-huge, more precisely the construction of the minimum model of ADℝ generalizes to the level of Vλ+1. This allows us to formulate the indicated generalization of ADℝ and then to prove that if the axiom holds in V at (...) a proper class of λ then in every suitable extender model, the axiom holds at a proper class of λ. Kant's moral philosophy is grounded on the dignity of humanity as its sole fundamental value, and involves the claim that human beings are to be regarded as the ultimate end of nature. It might be thought that a theory of this kind would be incapable of grounding any conception of our relation to other living things or to the natural world which would value nonhuman creatures or respect humanity's natural environment. Scott Blanchard The morality of thought lies in a procedure that is neither entrenched nor detached. --Theodor Adorno Perhaps no author within or outside of the canon of Western literature wrote as extensively on the topic of solitude as did Francesco Petrarch. While many of our modern associations with (...) the term may find some of their origins traceable as far back as Petrarch's career, the term "solitude" carried with it much different associations in his century than it does for us today, influenced as we are by the Romantic tradition and especially by Wordsworth's absorption with the idea of solitude as an ideal state for self-conscious reflection on the individual's personal history and its meaning. No less than three of Petrarch's major prose works--the De otio religioso, the De vita solitaria, and the Secretum--confront the topic of solitude directly, and we can also point to many letters, poems, and portions of his treatises that either directly or indirectly reflect Petrarch's virtual obsession with solitude. In Petrarch we are confronted with an author who was at once the most politically connected intellectual of his century--we are even tempted to call him a "public intellectual"--and yet who constantly invoked his desire for solitude, for an escape from the many distractions and seductions that the cities and courts of his society offered. For Petrarch these were invariably places where desire lurked and power held sway, so that the life of solitude was often invoked as an alternative--a counter-cultural locus, perhaps--where freedom from temptation and subjection was imagined as an escape from the negotium of worldly life. There is much in Petrarch's writings on the solitary life that is wishful and that we might characterize as merely compensatory in a life that was otherwise quite active and engaged, but there is also a [End Page 401] strain of thought in these works that indicates a certain intransigence beneath Petrarch's overdetermined attachment to the idea, if not the practice, of withdrawal and detachment. An inquiry into Petrarch's idealization of the solitary life may even yield a somewhat reactionary aspect of his complex mind, a conservative strain in an author who was in almost every other way the literary herald of a humanism that was at the center of the cultural avant-garde of the Trecento. Is the Petrarch that in so many accounts of his character is construed as the "first modern man" actually something else, a reluctant ascetic holdover from the Middle Ages? How did Petrarch use his unique understanding of the life of solitude, of ascesis, as a pose from which to construct his sense of himself as an author and intellectual? Or are we simply confronted in the figure of a worldly and an ascetic Petrarch with yet another set of contradictions so appropriate to the "immensely complex" author whose poetic signature was the oxymoron? 1"Asceticism" and "solitude" are of course not synonymous terms, but any examination of a number of Petrarch's works as they fall within what might be called a genealogy of asceticism must recognize from the start that Petrarch's brand of asceticism was somewhat unique, especially insofar as it was deeply influenced by ancient Stoicism as well as by his reading in authors from the early Christian era. Consumer buying behavior is an important aspect in every marketing strategy to produce maximum output from the market. This study aims to determine how advertisement affects consumer buying behavior and brand loyalty by considering a mediator between brand awareness and the moderating role of perceived quality. For this purpose, this study targets the rising cosmetics industry. This study used the purposive sampling technique to collect data from 300 respondents with the help of an online survey method via Google doc. The (...) partial least squares structural equation modeling PLS-SEM was applied to verify the hypotheses relationships. The findings have confirmed that advertisements substantially predicted brand awareness, brand loyalty, and consumer buying behavior. Furthermore, brand awareness partially mediated the association of advertisement with brand loyalty and consumer buying behavior. Also, perceived quality is significantly moderated on the association of brand awareness with brand loyalty and consumer buying behavior. Based on such findings, this study has contributed to the literature and provided new insights into the practical implications alongside the future roadmap of the survey. (shrink)

I contend that Jews, Christians, and Muslims who seek peace should not be reluctant to acknowledge the existence of their sometimes profound disagreements, or to affirm the truth of their own beliefs and practices. Since this places me at odds with John Hick, I analyze his views, granting the strengths of his critical realism and arguing that his revisionist-pluralist theory of religion has significant limitations for interreligious dialogue. Since the veridical-pluralist alternative I propose facilitates rather than stifles disagreement, I examine (...) three different conceptions of dialogue that acknowledge the importance of disputes among those searching for truth. I also discuss three virtues--honesty, empathy, and humility--whose cultivation would make us less likely to fail in our search for truth and the peace that is its sign and fruit. (shrink)

The purpose of this article is to call for the formulation and adoption of a declaration on the universal rights and duties of business. We do not attempt to define the specific contents of such a declaration, but rather attempt to explain why such a declaration is needed and what would be some of its general characteristics. The catalyst for this call was the recognition that even under optimal conditions, good companies sometimes are susceptible to moral lapses, and when companies (...) undertake ventures in authoritarian countries with poor human rights records, even those with the best intentions may find themselves drawn into complicity in human rights abuses. There, market exigencies may persuade them to leave their codes of ethics and commitments to human rights at home. Pragmatism, it would seem, requires thatthey accept the ethical inconsistencies that follow from a “When in Rome, do as they Romans do” outlook. When facing the moral dilemma about whether or not to invest in human rights abusing countries, companies are offered two alternatives: they can operate in those countries and accept potential complicity or they can stay away. We suggest, however, that a preferable option is to address the underlying problem, and to this end we advocate the promulgation of a declaration for business that is comparable to the Universal Declaration of Human Rights. Our proposed name for this is the “Declaration on the Universal Rights and Duties of Business.” To be effective, such a declaration would require enforcement mechanisms. To illustrate this issue, we focus on China, not because China is unique in its human rights abuses, but because China occupies such a central place in the globalization efforts of the major companies from around the world. A robust international declaration is needed to provide a common framework for the practice of consistent and fair business competition everywhere. (shrink)

Sir arthur edington's brilliantly phrased article, “Physics and Philosophy,” which appeared in the January 1933 issue of Philosophy, seems to me to contain a number of things which are calculated to be provocative to the mere philosopher. And I propose in this article to discuss what appears to be one of the most important of these provocative things, namely, Sir Arthur's view of the status of the physical world.

This paper starts from the remarkable habit of yoik singing among the Sámi people and the notion of togetherness in ‘singing together’, being confronted with the role of technology in the contemporary manifestations of ‘singing’. An important link appears to exist between several forms of grooming and the social dimensions of language and singing, as previously shown by Robin Dunbar. These types of interaction surpass the physiological, hormonal and psychological roles of grooming in humans, in non-human primates and in other (...) mammals. For they also determine the constraints of group-forming processes in mammals, and social structures in humans too. The characteristic group size of grooming mammals has been reported to determine the necessity of social stratification. Number constraints therefore form a silver lining throughout this paper. The analogy with artificial language models, and modern language models (MLM) in particular, has been chosen following the suggested applicability of AI for the deciphering of the language of Whales (Cetaceans). The polemic discussion between Noam Chomsky’s so-called classical generative linguistics and propagators of the nonlinguistic MLM approach (referring to Steven T. Piantadosi) is found an instructive heuristic for analyzing the future impact of AI on human culture. It is suggested that AI-generated analogues of ‘singing together’ will profoundly interact (or already do so) with social stratification and with the conservation of human values. (shrink)

The subject which has been chosen for me is sufficiently comprehensive. Several years ago I wrote the last of a series of essays in a book called Science, Religion, and Reality , in which, as requested, I tried to sum up the contributions of the other writers, with reflections of my own. I have also given a short statement of my opinions in the first volume of that interesting book, Contemporary British Philosophy . Lastly, I have tried, in a book (...) published in the autumn of 1933, to consider the religious and philosophical implications of recent scientific theories and discoveries, and particularly of the conviction held by our leading astronomers and mathematicians that the Second Law of Thermodynamics is unassailable, so that the ultimate extinction of the universe as we know it is certain. I showed that the acceptance of this verdict raises important questions for the philosopher and theologian. (shrink)

BackgroundPsychological distress/morbidity is amongst the primary reason for the cause of pain at multiple sites, its progression, and recovery. Though still not very clear if physical pain in the neck or the back may predict psychological morbidities or not. Thus, we investigated the association between combined neck or back pain and psychological distress/morbidity.MethodsA cross-sectional study was conducted in Al-Kharj, Saudi Arabia, including 1,003 individuals. The questionnaire comprised of General Health Questionnaire-12 and some questions about neck and back pain. Data analysis (...) was done using statistical software SPSS version 26.0.ResultsThe results of the multivariate analysis revealed a significant positive association between neck/back pain status and total GHQ score. Having neck/back pain had almost a 2.5 times greater risk of psychological distress/morbidity. Further, females were more likely to have a higher risk of psychological distress/morbidity than males while adjusting for sociodemographic and clinical characteristics.ConclusionThe combination of neck and back pain was significantly associated with the Saudi population’s psychological problems. Therefore, the Saudi government needs to devise high-risk strategies and allocate adequate resources to the cause so that at-risk people can be shielded from the adverse complications arising from this condition in the long run. Low heart rate causes a risk of death, heart disease, and cardiovascular diseases. Therefore, monitoring the heart rate is critical because of the heart’s function to discover its irregularity to detect the health problems early. Rapid technological advancement allows healthcare sectors to consolidate and analyze massive health-based data to discover risks by making more accurate predictions. Therefore, this work proposes a real-time prediction system for heart rate, which helps the medical care providers and patients avoid heart rate risk in real (...) time. The proposed system consists of two phases, namely, an offline phase and an online phase. The offline phase targets developing the model using different forecasting techniques to find the lowest root mean square error. The heart rate time-series dataset is extracted from Medical Information Mart for Intensive Care. Recurrent neural network, long short-term memory, gated recurrent units, and bidirectional long short-term memory are applied to heart rate time series. For the online phase, Apache Kafka and Apache Spark have been used to predict the heart rate in advance based on the best developed model. According to the experimental results, the GRU with three layers has recorded the best performance. Consequently, GRU with three layers has been used to predict heart rate 5 minutes in advance.
